A Medical School student's University of Wisconsin-Madison academic record lists all courses taken and grades earned at the University. This academic record is on file at the Office of the Registrar, and UW-Madison student records and official transcripts may be obtained at 21 N. Park Street, Suite 7223, Madison WI, 53715-1218.
Detailed information concerning a student's grades, relative class standing and clinical performance is available upon request of the student from the UW-Madison Medical School or the Registrar's Office.
